The Quarterly Relationship Check-In

The Quarterly
Relationship Check-In

A Private Self-Assessment for Couples

This audit is a private, individual self-assessment designed to be completed separately by each partner, then shared and discussed together. It is not a test, a grade, or a record of past wrongs. It is a structured way to put honest feelings into words, every 90 days, before small frustrations calcify into resentment, distance, or silence.

Most relationship breakdowns, including infidelity, do not happen overnight. They happen in the unspoken gap between what one partner feels and what the other partner knows. This check-in closes that gap on a regular schedule, before it becomes a crisis.

Each partner fills this out alone and honestly. Then you exchange answers and talk. The goal is not to win an argument or prove a point. The goal is to build one accurate, shared picture of where the relationship stands right now, so you can decide together what to build next.

Instructions: For each statement, mark the response that best reflects how you honestly feel right now, then use the space below it to explain why, give an example, or add anything the scale can’t capture. There are no wrong answers here, only honest ones.
